Without information, including consulting stakeholders and donors to understand their needs, views and expectations
Which results in poorly structured or unfinished plans
When you are too sure of yourself

Done by one person or without input from the people who have to implement the plan (staff, volunteers)
Without considering your plan's audience
Done because it "needs to be done", or because someone has asked for it
Written by people who don't understand your organization
That makes results impossible to prove
